Your initial visit will include a review of your health history, pertinent x-rays, a thorough oral exam, and diagnosis/ treatment planning. Please assist us by providing the following information at the time of your consultation:
- A panoramic x-ray taken within the last year (if available) from your general dentist or orthodontist. Otherwise we can take a digital panoramic x-ray during your initial visit.
- A list of medications you are presently taking.
- Any dental and/or medical insurance cards and information.
Should you choose to schedule treatment, the details of your surgery and anesthesia options will be explained. A surgical assistant will go over the pre- and post-operative care instructions. Financial options will be discussed and any questions you have will be addressed. You will be asked to sign an informed medical consent document as well as a financial form. Written instructions will be given to you to take home for review.
