New Patient Registration
You may preregister with our office by filling out our secure online patient registration form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Your initial visit will include a review of your health history, pertinent x-rays, a thorough oral exam, and diagnosis/ treatment planning. Please assist us by providing the following information at the time of your consultation:
A panoramic x-ray taken within the last year (if available) from your general dentist or orthodontist. Otherwise we can take a digital panoramic x-ray during your initial visit.
A list of medications you are presently taking.
Any dental and/or medical insurance cards.
Should you choose to schedule treatment, the details of your surgery and anesthesia options will be explained. A surgical assistant will go over the pre- and post-operative care instructions. Financial options will be discussed and any questions you have will be addressed. You will be asked to sign an informed medical consent document as well as a financial form. Written instructions will be given to you to take home for review.
Our treatment and financial coordinators make every effort to optimize your insurance benefits. Colorado Surgical Arts is contracted with the following insurance providers:
Dental Insurance: Delta, Cigna, MetLife, United Healthcare, Humana, and Aetna
Medical Insurance: Anthem BCBS, United Healthcare, Cigna, and Aetna
With your consent, the practice is permitted by federal privacy laws to make uses and disclosures of your health information for purposes of treatment, payment, and health care operations. Protected health information (PHI) is the information we create and obtain in providing our services to you. Such information may include documenting your symptoms, examination and test results, diagnoses, treatment, and applying for future care or treatment. It also includes billing documents for those services.
